At Unique Pediatric Hearts (UPH), our mission goes far beyond routine medical care for children with complex medical needs. We’re dedicated to creating a holistically safe, nurturing, and compassionate environment for both children and the families who love them. 

Founded by nurse and owner Emily Swift, UPH provides a full spectrum of pediatric home care services throughout Georgia including skilled nursing, personal support services, school and therapy support, and respite care. Whether families need 24-hour nursing assistance, daily personal care, or specialized support for therapy and education, UPH delivers compassionate, high-quality care right where it’s needed most: at home.

More than just a care provider, UPH is a calling, a family, and a trusted support system rolled into one. Every service we offer is rooted in one mission: to deliver care that’s as compassionate as it is clinical, helping children thrive and families find peace of mind throughout Georgia.


A Call to Care for Children

Emily’s journey began with a deeply personal experience that shaped her entire approach to pediatric care. 

“There was a boy I babysat when I was younger, and he had terminal cancer,” she recalls. “He told me he needed me to call his mom because it was time for him to go to sleep. I didn’t know he meant he was going to pass away, but I stayed with him. I was able to give that family peace knowing he wasn’t alone.”

That moment ignited Emily’s lifelong dedication to children facing life-limiting illnesses. Every child who comes into her care receives the same level of attention, love, and respect. “I want to give a hundred percent of my dedication not only to the child but to the family as well,” she says.


Nurses Chosen for Skill and Heart

At UPH, our nurses are carefully selected not only for their medical expertise but for their compassion, empathy, and integrity. Nikki Estes, a nurse with UPH for more than five years, embodies that spirit.

“Sometimes it’s hard to accept help,” Nikki shares. “But Miss Emily would look at me and say, ‘Sit down. That’s my job. I got this.’ I really believe this type of work is a calling. The children brighten up my day. You can teach technique, but you can’t teach compassion, and that’s what sets our care apart.”

For Nikki and the team, care goes beyond standard medical procedures. It’s also about building relationships. “I end up being like family with my clients,” she explains. “They know me as Nurse Nikki, and I’m excited to see them. They’re excited to see me.”


Whole-Family, Whole-Hearted Care

At UPH, care extends far beyond the patient. The team recognizes that families of medically fragile children face immense stress, and sometimes these families need more than a nurse—they need a partner.

“Sometimes we’re counselors. Sometimes we’re a person for them to vent to. Sometimes they’re so tired, they just need us to run to the grocery store,” Emily shares. “It’s about taking care of the entire family, not just the patient.”

Through our personal support services, caregivers assist with daily activities like feeding, bathing, and mobility, providing both relief and reassurance for parents. Our respite care services give families the time they need to rest and recharge, knowing their child is in compassionate, capable hands.


School and Therapy Support: Helping Kids Thrive

Every child deserves the chance to grow, learn, and participate fully in life. That’s why we offer school and therapy support, partnering with families, educators, and therapists to ensure children can safely take part in learning and development.

Our nurses and care team members assist with in-home therapies, accompany children to school or appointments, and collaborate with educational staff to create an inclusive environment.

“Even though she was assigned to Sariah,” one parent shared, “our nurse didn’t just focus on her: she always included the other kids and helped them connect. She made everyone feel seen and valued.”

We approach care as a whole system, not just medicine. To us, care is about connection, inclusion, and helping children thrive in every setting.

“It feels just like a family,” Nikki says. “I love working with my patients and my coworkers. I’m excited every day to do my job.”

Emily ensures that help is always just a phone call away. “Twenty-four–hour nurses are available. I always answer my phone. If I miss a call, I respond within five minutes or return that call within 24 hours. We’re here for the families whenever they need us.”

UPH understands from experience that caretakers of medically complex children face uniquely challenging circumstances, whether that means navigating complicated care systems, coordinating doctors’ appointments, or simply putting dinner on the table.

“There’s not a lot of people left with that caring heart,” Emily says. “I try every day to think positively and do my best—for the children, the families, and the world around us.”
